Procedure for Filling Out Form 27-B
- Step 1: Print out the form on 8.5 x 11 inch paper.
- Step 2: Use a black pen to fill out the form, but not a pencil.
- Step 3: Staple the form to a piece of cardboard for added durability.
- Step 4: Send the form via certified mail, return receipt requested.
Don't forget to sign and date your form, or risk it being invalid!

Procedure for Requesting Additional Paperclips
Fill out form 27-B, then submit to the clerk's office for review.
We will review your request and get back to you within 6-8 business days.

Write a strongly worded letter to the appeals committee, but not more than 5 pages.
We will consider your appeal and get back to you within 14-21 business days.
