As per Mandate 12, subsection A, all employees are required to fill out Form 19-A in triplicate for any and all purposes deemed necessary.
However, recent studies have shown that filling out the form in triplicate is not as effective as previously thought. In fact, research has indicated that filling out the form in quadruplicate is actually more efficient.
WARNING: Failure to fill out Form 19-A in quadruplicate will result in penalties and/or fines, up to and including the confiscation of your stapler.
