Wherein lies the tale of the probability system that refused to work

It started innocently enough, a simple 20 sided die, a fair 6 sided die, and a d12 for good measure. But then things went south. The 20 sider started rolling 1s every time, the 6 sider refused to roll above 3, and the d12 just wouldn't roll at all. It was a probability nightmare.
