Step 1: Gather all required documents, including but not limited to: birth certificate, marriage certificate, divorce decree, and proof of vaccination.
Step 2: Find the nearest forms office and wait in line. If you're having trouble finding it, ask a local.
Step 3: Fill out Form 27-B, carefully checking off each box with a #2 pencil. If you make a mistake, start over from scratch.
