Level 1: Initial Contact

When encountering a problem, please follow these steps:

  1. Fill out Form 27-B: "Problem Report" in triplicate, using 12-point font and black ink.
  2. Submit Form 27-B to the designated authority figure (usually someone in Accounting).
  3. Wait 47 business days for a response.
