Section 3, Subsection 3-2, Article 1: All forms must be completed in triplicate, using only black ink, on 8.5 x 11 inch paper, with no staples, tape, or other forms of binding.
Section 3, Subsection 3-2, Article 2: All forms must be submitted within 14 calendar days of receipt, or a late fee of $500 will be assessed.
