Step 1: Fill out form 27-B/6. Make sure to use a black pen, as colored pens are not accepted.
Step 2: Attach all relevant documents, including but not limited to: birth certificate, social security number, and proof of vaccination.
Step 3: Wait 6-12 months for a response from the Bureaucratic Committee of Infinite Delays.
Step 4: Receive a letter informing you that your application has been misplaced, and that you must start over.
