Appendix B: The Official Guide to Correctly Filing a Form 345-B/6

Step 1: Gather all necessary paperwork, including but not limited to:

Form 345-B/6
Form 345-B/7 (optional, but recommended for maximum clarity)
Two (2) pens, preferably in working condition
One (1) calculator, preferably not the one from 1987

Step 2: Fill out Form 345-B/6 in its entirety, using only the black ink provided.

Step 3: Attach all required supporting documents, including but not limited to: a self-addressed stamped envelope, a copy of your drivers license, and proof of citizenship.
