Don't bother with fancy forms when you have the essentials.
Here are the must-haves for every form:
- Full Name (or Nickname, Handle, or Alias)
- Birthdate (or preferred Age Range)
- Occupation (or Hobby/Interest)
These are just the beginning. You can always add more fields, but don't bother us with that.
Additional Form Fields to Make Life Interesting
Optional but oh-so-optional fields for the thrill seekers:
- Favourite Food (or Drink, or Both)
- Preferred Mode of Transportation (or Mode of Transportation-related Trauma)
- Number of Socks in your collection (or closest estimate)
