This form, also known as the "Bureaucratic Nirvana Form," must be filled out by hand in triplicate and submitted to the Bureaucratic Review Board.
Exceptions to the Tuesday submission rule are only made for leap years, when the bureaucrats are allowed to submit their forms on a Wednesday.
