Section 1: Important Information
As per the Bureaucratic Code of Conduct, Article 3, Subsection 7, it is a requirement that all forms be filled out in triplicate, using only approved forms 12, and signed in the presence of at least two witnesses, one of which must be a member of the Bureaucratic Union.
Furthermore, all forms must be submitted within 30 minutes of filling out, or else the form is deemed null and void.
Section 2: Supporting Documents
A copy of your birth certificate, a letter of introduction, and a notarized affidavit of authenticity must be attached.

Section 3: Submission Guidelines
Please submit your form to the nearest Bureaucratic Outlet, or via email to [bureaucracy@assurance12.gov](mailto:bureaucracy@assurance12.gov).
Do not submit via carrier pigeon, as our IT department has not yet upgraded to 2.0.
