Declaration 21c: The Unabridged Edition

Section 1: Preamble

In accordance with the dictates of the Bureaucratic Code, it is hereby declared that all forms must be filled out in triplicate.

Whereas, it is the finding of this noble assembly that the current form 21a was, in fact, a travesty.

Be it therefore resolved, that form 21c shall be the standard form for all purposes henceforth.

Section 2: Article I - Form Layout

The form shall consist of the following sections:

1. Section A: Personal Info
2. Section B: Bureaucratic Clearance
3. Section C: Itemized Expenses

Section 3: Article II - Section A

Section A: Personal Info shall contain the following fields:

1. Name:
2. Address:
3. Phone Number:

Section 4: Article III - Section B

Section B: Bureaucratic Clearance shall contain the following fields:

1. Bureaucratic ID Number:
2. Clearance Level:

Section 5: Article IV - Section C

Section C: Itemized Expenses shall contain the following fields:

1. Expense Item:
2. Amount:

And so on, and so forth.
