Description: The Phase One Bloopocalypse was a catastrophic event caused by an untested patch of JavaScript that went rogue.
Causes: A team lead's questionable coding choices
Damage: 3 servers, 1 sanity
Our team's investigation found that the Bloopocalypse was caused by a single line of code that looked suspiciously like it was written by a 5 year old.
The code in question was: "if (1 == 0) { alert('BOOP'); }"
The team recommends that all team leads be required to attend Code Review 101 before being allowed to write any code.
All future code reviews should include a panel of experts in the field of "Not Writing Bad Code"
