Standard-Issue Procedure 4 (SIP-4)

For the most efficient processing of your claim, please follow the instructions below:

1. Fill out SIP-4 Form (pdf) in duplicate and triplicate.
2. Attach three (3) original and three (3) photocopies of your supporting documents.
3. Return the completed form and attachments to prophetsประก Department of Red Tape by the 15th of the month.
4. Wait for 6-8 months for a response. Or 12-18 months for a response from our automated response unit.
